Hosts unable to receive address via dhcp

I have an issue whereby some hosts are able to obtain addresses and others can't. All my hosts are on VLAN.1 at the moment. My config is attached


 

Pls assist.

If you move the hosts that don't pick up an IP from DHCP to a new port does it work?

Your config looks like it should work fine.  Is it possible that there is so much traffic on a link that you are seeing storm-control drop the DHCP (broadcast) traffic?  I might also try a newer release of the 10.4 code train.  Current software is 10.4R9, and it appears you are still running 10.4R1.9.  Note that the upgrade from 10.4 pre R2 to something after R2 requires a boot-loader upgrade as well.
