    Hello akushner,

         Thank you for your reply,

I tried to use the install-media file to format the installation using a USB stick.

However, still getting an error as follows:

***** FILE SYSTEM WAS MODIFIED *****
mount_cd9660: /dev/da2s2a: No such file or directory
    ERROR: discover_install_drive: /mnt/tmp/preinstall/install.conf not found

This kb notes the issue and possible recovery paths.

    Hello spuluka,

Thank you very much for your reply,

By formatting the installation, you will still be prompted with the following error

***** FILE SYSTEM WAS MODIFIED *****
mount_cd9660: /dev/da2s2a: No such file or directory
    ERROR: discover_install_drive: /mnt/tmp/preinstall/install.conf not found
You are now in a debugging subshell (you may not see a prompt)...

Looks like you will need to open a JTAC case.  The kb notes that if these recovery methods don't work then a case is required proceed.  I would suspect there is a hardware problem at this point.
